Important Notice — Form Validity
If any gap, box, slot, or field is left blank or unsigned, your application will be regarded as INVALID / INCOMPLETE and may be declined. If any field is not applicable to you, you must write "N/A" or "Not Applicable". Failure to submit required documents will also make your application invalid.
1. Applicant & Employment Details
When applying, you will be required to provide complete personal and employment records:
- Full Name & Surname
- Cellphone & ID Number
- Residential Address
- Date of Birth & Marital Status
- Spouse Name (if married)
- Occupation & Work Telephone
- Years of Service & Basic Salary
- Banking Details & Account Number
2. References & Next of Kin
Two verifiable contacts must be supplied for security and contact confirmation:
Next of Kin (Primary Contact):
Full Name, ID Number, Cellphone & Alt Number, Relationship, Residential Address, Occupation & Workplace.
Second Contact Person:
Full Name and Cellphone Number.
3. Required Documents & Terms
Interest Tiers: Short Term is fixed at 40% per 30 days. Long Term options: 60 Days = 75% Interest (2 Installments), 90 Days = 110% Interest (3 Installments). Further interest is added to any balance overdue until zero balance is achieved.
Clearance & Default: Instant transfer fee of R50 applies for non-matching banks. Defaults carry a R50 daily fee until paid.
4. Working Hours & Response Time
Applications are reviewed and disbursed in orderly sequence during operating hours:
Applications received outside working hours will be attended to promptly on the next business morning.
